Reporting Information For Physicians, Laboratories And Hospitals

The Physician/Hospital Report of Communicable Diseases is the form used to report communicable diseases. The following forms can be downloaded in Adobe PDF format or Microsoft Word. If you do not have an Adobe PDF Reader, you can download the appropriate version at the Adobe Web Site or contact The Miami-Dade County Health Department at (305) 470-5660 and request a form.
