To receive services from the Florida Department of Health ADAP Program, a patient must complete 2 steps:

1. Print, complete and sign the CORE ELIGIBILITY APPLICATION & CONSENT TO FAX forms
2. Attach the required Supporting Documentation
3. Take the two forms and the required documents to your case manager.
