How to Report a Case of TB



A report should be made to the Surveillance Section of the TB Program when:

  • A laboratory confirms TB disease is present
  • TB disease is suspected based on clinical or radiological examination of the patient
  • A client is started on two or more anti-TB medications

Follow the link to the Florida Department of Health TB Reporting Requirements for Health Care Providers.  Please refer to the CDC Case Definition for TB with any questions about identifying a potential case of TB.
Healthcare providers are required by law to report any individual being treated for TB.  Florida Statute 392.53 requires reporting of this information to the local health department.

A report of TB may be made either by telephone to (305) 324-2470 or by Fax to (305) 547-5571.

When notifying the Miami-Dade County Health Department about a suspected or confirmed case of TB, please complete and submit the MDCHD Case/Suspect Report.  Feel free to contact the TB surveillance unit with any questions about completing this form.  Provide as much detailed information as possible.  Completed reports should be faxed to (305) 547-5571. 

Upon receiving a report of TB, a TB Program Surveillance Analyst will contact the reporting physician or facility to obtain further information regarding the case.  One of the services offered by the MDCHD includes Case Management of TB patients.  Scheduling an appointment for a patient at one of our clinics can be discussed at this time.  However, if the patient will be followed by a private physician, a Follow-up Report on Patient with Tuberculosis detailing the patient’s progress with treatment and any changes in bacteriology & radiology must be submitted to the MDCHD bi-monthly.  All follow-up reports should also be faxed to (305) 547-5571.
